HOSPITAL PRE-ADMISSION

Once we have received your referral from your referring GP, you will need to complete the mandatory hospital Pre-Admission documentation prior to being admitted to the Southern Endoscopy Centre for your procedure. 

The documentation consists of a Patient Admission Form, Financial and Privacy Consent Form and a personal Health Questionnaire.

In order to minimise procedural delays, we request that you return the completed admission forms to us at least five business days prior to your admission.
